Help your patients meet their medical needs at home
Nurse At Your Door’s team of RNs and LVNs can provide your patients with case management and skilled nursing services in the comfort of their own homes. We can help your patients with both chronic and acute care on an as-needed or recurring basis. Our remote monitoring service will proactively alert you when your patient’s vital signs move outside of the range you’ve specified.
The health problems of millions of Americans are directly related to the patients’ noncompliance with physician recommendations, non-adherence the prescribed medication regimen and poor understanding of health related issues and potential poor outcomes. Nurse At Your Door improves your patients understanding of their medical issues and increases their compliance with the treatment and medication plan resulting in better medical management and better health outcomes.
NAYD is a perfect complement to traditional home health services that spend a limited amount of time per day and have a limited number of visits with your patient. We can see patients same day if needed or schedule them for routine visits.
